Accounting Forms
With IRS regulations continuing to tighten down on obtaining accurate social security numbers and verification of related information it is necessary for NCU to utilize a W-9 form in all situations to enable payments to individuals. W-9’s must be updated annually even if we have one on file from a previous payment.
This form is required any time an employee wishes their MN withholding to be different from their Federal withholding. It is also required if the employee claims ‘exempt’ on the federal W-4 form. Form should be turned in to ‘Payroll’.
